Each state finalist will turn in their forms on pageant weekend. This forms that must be completed and brought with you, ready to submit, to pageant check-In (i.e. your resume, emcee card, etc.). You may handwrite and use the Forms you were provided OR if you prefer to type out the information & print them off at home, you can do so here.

Do not mail any of these forms to your state pageant headquarters, you will bring them all with you to turn in at pageant check-in.

If you would like to have a physical copy, please email info@namoffice.com. Please put “Check-In Forms – Contestant’s Name” in the subject line of your email.

The forms will be the information that the MCs will read about you while you are on stage for the various overall and optional contests. There are also forms for Community Service, Art Contest, Academic Achievement, etc.

 

You only have the fill out the forms you need. Only Resume Form, Community Service Form, and Emcee Card are required for the overall pageant. You will need to fill out and print the corresponding form(s) for any optional contests you are participating in.
